We are OakMed Family Health Team

The OakMed Family Health Team is a not-for-profit corporation governed by a community board consisting of seven directors. The Oakmed FHT works with the physicians of the Oakville Family Health Organization FHO to provide comprehensive primary care services to the citizens of its community.

Physicians

Doctors are the backbone of our team and they evaluate symptoms, run tests to diagnose a patient’s condition and may prescribe medication or treatment to remedy the illness or injury. Primary care physicians are the patient’s most important partner in their healthcare and healthy life status.

Interdisciplinary Health Professionals (IHP)

Nurse Practitioner (NP)

A Nurse Practitioner provides the full spectrum of primary care to patients of all ages with a focus on preventing disease, promoting good health and curing illness. Nurse practitioners are registered nurses with extra education and advanced training who are able to diagnose, order tests and prescribe most medications.

Registered Social Worker (RSW)

Social workers focus on improving the health and social well-being of individuals, families, groups and communities. They work with people to assess, resolve, prevent or lessen the impact of psychosocial, physical and mental health related issues. They do counselling, therapy and deliver programs in collaboration with other professions.

Registered Dietitian (RD)

Registered Dietitians are your trusted health professionals in food and nutrition! They are uniquely trained to translate the latest evidence and research into understandable and practical nutrition recommendations to improve your health or manage your medical conditions.

Pharmacist

Pharmacist responsibilities include a range of care for patients, from dispensing medications to monitoring patient health and progress to maximize their response to the medication. The Pharmacist reviews medication, consults with physicians in selecting the best treatment, monitors side effects and drug interaction and makes the necessary changes, and educates patients on their medication.

Clinic Nurse

Our nurses provide and coordinate patient care, performs tests and measurements, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members. They work with physicians to perform examinations, deliver prescribed treatment and administer medications. Our nurses also provide messages and communications to patients concerning their health status and test results.

Administrative Team

Clinic Manager

The Office Manager is accountable for the daily operation of the organization. This includes; all human resources and business activities; scheduling of staff; customer service; computer and phone systems; and facilities. The Office Manager works with the Executive Director and the Lead Physician to ensure that the FHT and clinic is operating effectively and meeting its goals and the needs of the patients it serves.

Administrative Staff

Work with patients, staff and physicians undertaking multiple tasks and responsibilities that are critical to the functioning of the organization. They help book appointments, greet patients, prepare exam rooms, telephone follow-up, assist with coordinating meetings and a variety of other tasks to support the clinical activities.

Quality, Planning, and Information Management Lead

Reporting to the Executive Director, the Quality, Planning, and Information Management Lead is responsible for supporting OakMed in their quality improvement planning, decision making and implementation activities. This includes the implementation of data quality initiatives, improving the flow and use of information, developing queries and analytical products that support boards and leaders in their quality improvement goals.  This role also provides leadership, problem solving, relationship building, and general support.

Executive Director

The Executive Director is responsible to the Board of Directors of the FHT to advise on matters of policy and risk, quality monitoring, evaluation and health human resources. The ED develops, manages and maintains the budget, is responsible to manage human resources issues and develop plans around recruitment and retention. This role develops strategic plans and acts as a liaison with the community and the Ministry of Health and Long Term Care.
